Chinese medicine terminology committee. Terminology of traditional Chinese medicine (20 10)]

3 Summary Yang edema [1][2] is the name of disease syndrome [3]. One of the two main types of edema disease [3]. It refers to an edematous disease characterized by invasion of foreign pathogens, obstruction of meridians and acupoints, or obstruction of qi movement, in which the face swells first, with obvious swelling potential, and the body is weak when depressed [1]. Most of them are empirical [3]. Treatment should be based on eliminating pathogens [4].

"Danxi Heart Therapy for Edema": "If the whole body is swollen, the mouth is dry, the urine is red and the stool is closed, this is yang water."

"Danxi Heart Edema": "If the yang disease water is accompanied by yang syndrome, the pulse is heavy ... If the body is swollen, thirsty, the urine is red and astringent, and the stool is blocked, it belongs to yang water. First drink it with five skins powder or four grinds, add ground bitter orange, and then dilute it with water. "

Introduction to medical edema: "yang water is caused by external factors, wading in the rain, or seeing yang syndrome due to cold and summer dampness."

4 Definition of Yang Shui "Terminology of Traditional Chinese Medicine" (2004): Yang Shui refers to an edematous disease characterized by swelling of the face first, obvious swelling potential, depression at the touch of a button, and weakness of the body [2].

Terminology of traditional Chinese medicine (20 10): Yang water refers to an edematous disease characterized by the invasion of foreign pathogens, the obstruction of acupoints, or the obstruction of qi movement, which causes the body fluid to overflow. The face swells first and has obvious swelling potential, and the main manifestation is depression [1].

Yang water is one of the two major types of edema disease [3].

The etiology and pathogenesis of yang water are mostly positive [3]. It is caused by exogenous wind, cold, dampness, heat and summer heat [3]. Introduction to medical edema: "yang water is caused by external factors, wading in the rain, or seeing yang syndrome due to cold and summer dampness."

6 Shui Yang's "Danxi Heart Edema" Symptoms: "If the whole body is swollen, dry mouth, red urine and closed stool, this belongs to Shui Yang."

Acute attack of wind pathogen on lung, first edema, then the whole body, is not conducive to urination; Those who are cold are cold without sweat, with white and slippery fur and tight pulse; Those who are prone to wind-heat have sore throat, thin yellow fur and floating pulse.

Damp pathogen trapped spleen, edema all over the body, especially edema of limbs, short and red urine, mental fatigue, loss of appetite, chest tightness, white and greasy fur and slow pulse.

7 dialectical treatment of yang water Yang water treatment should be sweating, diuresis, tapping [3].

7. 1 Yang Shui Fang, appropriate sweating, diuresis, tapping. Lung qi does not rise, triple energizer stagnates, water cannot enter the bladder, and symptoms such as aversion to cold and fever, cough and sore throat, facial edema, short and red urine, thin fur and floating pulse appear. , should be dispersing wind and dispersing lung, clearing heat and promoting diuresis, such as Bai Yue soup and Wupi drink. If you see edema, upset, thirst, red and astringent urine, constipation, full abdominal distension, yellow fur and yellow pulse, you should clear away heat and drain water, and drink it with Bazhengsan and Shuchi. [3]

One-by-one tapping method is a very common method to treat yang edema. If used properly, it has an immediate effect, but it depends on the needs of the disease. Generally speaking, soon after the onset, the swelling is very serious and the healthy qi is still flourishing. At this time, we should pay close attention to the pathogenesis, take eliminating water as the top priority, and choose appropriate drugs to attack water, so that evil water can quickly stay away from defecation. Once the water recedes, we can discuss the tonic later. In the late stage of the disease, both spleen and kidney are deficient, and edema is particularly serious. If the disease breaks out, although the water recedes for a while, it is easy to hurt and the root of the disease is not removed. When the water evil comes back, it will be more fierce and sicker. As pointed out in "Danxi Heart Therapy for Edema", "Don't use Daphne genkwa, Euphorbia, and Kansui violently." Therefore, drugs should be used with caution.

7.2 Acupuncture treatment of yang water 7.2. 1 Prescription and principle of one treatment: expelling wind and dispersing lung, invigorating spleen and promoting diuresis.

Prescription: Shu Fei lacks the triangle tree Shuiyinling Spring.

Fang Yi: The upper part is swollen. It should be distributed. Shu Fei is the place where the qi of the lung meridian is transferred, cooperating with the lung collaterals, dispersing the lung and passing water. Sanjiaoshu regulates gasification function; Water control can be divided into clear water and turbid water; Yinlingquan is a point of spleen economic cooperation, which can strengthen the spleen and promote diuresis.

Operation: the needle is opened once a day 1 time for 30 minutes, and1time is a course of treatment.

7.2.2 Prescription 2: Take Sanjiaoshu, Shuishui, Qihai, Zusanli and Yinlingquan as the main points [5].

Matching points with syndrome: Shui Yang Jiayang Lingquan, Fuliu and Hegu; Facial swelling is very serious in the middle and front roof [5].

7.3 Yang water moxibustion therapy [6]

Acupoints: Shuishui, Shu Fei, Sanjiaoshu and Yinling Spring.

Moxibustion method: Moxibustion with ginger-separated mugs, with half an olive-sized mugs, each point 10 is strong, and when the skin feels burning, remove it, once a day or every other day, 1 0 is 1 course of treatment, until the edema subsides and the symptoms disappear.

7.4 Feng Shui Syndrome: Yang EDM with Feng Shui Interjunction Syndrome [1] refers to wind evil invading the lungs, abnormal performance, water overflowing the skin, head and face eyelid edema, and then affecting the whole body, with shiny skin, easy recovery, rapid onset, aversion to cold and fever, anhidrosis and sore throat.

7.4. 1 yang-water-feng shui syndrome symptoms: eyelid edema, followed by swelling of limbs and whole body, bright skin, easy recovery by pressing, rapid onset, aversion to cold and fever, sore limbs, dysuria and other symptoms. Those who are prone to wind-heat are accompanied by sore throat, red tongue and slippery pulse. People with wind-cold aversion to cold, cough and asthma, thin and white tongue coating, and slippery or tight pulse. If the edema is serious, you can also see deep veins. [8][9]

7.4.2 Syndrome analysis: wind evil attacks the exterior, the lungs do not fall, the waterways are blocked, and the bladder is sparse, so symptoms such as wind evil, fever, limb aches, dysuria, and systemic edema are seen. The lung position is focused, the hair is blocked, and the water is stagnant, so edema should be seen in front of the eyelid head; If the law is not correct, it is not conducive to blasphemy. Water and body fluids can't be transported, overflow the skin and immediately spread throughout the body. Wind belongs to yang evil, feng shui struggles, and the skin is bright and easy to sag. Symptoms such as fever, sore throat and cough can be seen due to pathogenic wind. If wind pathogen is accompanied by heat, the throat is red and swollen, the tongue is red and the pulse is slippery. If the pathogenic wind is accompanied by cold, the pathogenic wind is on the muscle surface, the defensive yang is restrained, and the lung qi is not clear, then you will see aversion to cold and fever, cough and asthma. If the swelling is serious and the yang is suppressed, you will see a heavy pulse, or a slippery pulse, or a heavy pulse. [8][9]

7.4.3 Prescription for Treating Yang-Water-Fengshui Syndrome [8]

Treatment: dispelling wind and clearing heat, dispersing lung and activating water.

You can use Yuemai Jiashu Decoction [Remarks] Yuemai Jiashu Decoction (Synopsis of the Golden Chamber): Herba Ephedrae, Gypsum Fibrosum, Radix Glycyrrhizae, Fructus Jujubae, Atractylodis Rhizoma, and Rhizoma Zingiberis Recens. In the prescription, Herba Ephedrae spreads lung and sweats to relieve exterior syndrome, so as to remove exterior moisture; Gypsum plaster can relax tendons and clear heat; Atractylodes macrocephala, Glycyrrhiza uralensis Fisch, Ginger and Jujube strengthen the spleen and eliminate dampness, which means worshiping soil and making water. Duckweed, Alisma orientalis and Poria cocos can be added as appropriate to help disperse the lung and promote diuresis to reduce swelling. If the throat is sore, Radix Isatidis, Radix Platycodonis and Fructus Forsythiae can be added to clear away the throat and dissipate the stagnation and detoxify; If the fever is heavier and the urine is less, fresh grass roots can be added to clear away heat and diuresis. Through the passage of water, Yinling Spring can strengthen the spleen and promote diuresis, so that the water in the water can be transported to the bladder when it is wet. Separation of exterior and interior can dispel wind and reduce swelling.

7.5 Syndrome of Wet Poison Invasion 7.5. 1 Shui Yang Symptoms of Wet Poison Invasion Syndrome: Eyelid edema, spreading to the whole body, dysuria, skin sores, even ulceration, aversion to wind and fever, red tongue, thin yellow fur, floating or slippery pulse [8].

7.5.2 Syndrome Analysis The skin is dominated by the spleen and lung, so the skin has scars. The dampness toxin failed to be cleared up and dissipated in time, and it belongs to the viscera, so that the spleen and stomach of the middle jiao can not transport water and dampness, lose the ability to ascend and descend turbidity, and make the lungs unable to pass through the water channel, which is unfavorable for urination. Wind is the length of all diseases, so it is often accompanied by wind evil at the beginning of the disease, which is manifested as eyelid swelling and spreading to the whole body quickly, and the evil wind is like fever. Its tongue is red, its fur is thin and yellow, and its pulse is floating or slippery, which is caused by wind evil mixed with wet poison. [8]

7.5.3 Treatment of dispersing lung qi and detoxifying, promoting diuresis and reducing swelling [8].

7.5.4 Prescription for treating Shui Yang and damp toxicity [Remarks] Mahuang Lianbian Chixiaodou Decoction (Treatise on Febrile Diseases): Mahuang, Almond, Raw Catalpa White Peel, Lianbian, Chixiaodou, Glycyrrhiza uralensis, Ginger, Jujube and Wuwei Disinfection Drink [Remarks] (Medical Disinfection Drink) Mahuang, Almond, Mulberry Peel, etc. Before, there were dispersing lung qi and promoting diuresis, forsythia suspensa clearing away heat and dissipating stagnation, and adzuki bean promoting diuresis to reduce swelling; Honeysuckle, wild chrysanthemum, dandelion, viola yedoensis and cabbage are used in the rear to enhance the power of eliminating dampness and detoxifying. If sepsis is very serious, dandelion and viola yedoensis should be reused; If wet and erosive, Radix Sophorae Flavescentis and Rhizoma Smilacis Glabrae are added; If the wind is itchy, add Cortex Dictamni Radicis and Fructus Kochiae; If blood heat is red and swollen, Cortex Moutan and Radix Paeoniae Rubra are added; If the stool is blocked, increase the yellow nitrate. [8]

7.6 Syndrome of Water-wet Immersion Yang EDM with Dam Retention Syndrome [1] refers to water-wet, skin-soaked, slow onset, long course of disease, edema all over, short and red urine, heavy weight, chest tightness, anorexia, fatigue, pale tongue and white fur.

7.6. 1 Symptoms of wet immersion syndrome of yang water: edema, scanty urine, heavy tiredness, chest tightness, anorexia, nausea, white and greasy fur, slow pulse, slow onset and long course of disease [8].

7.6.2 Syndrome analysis: The evil of water and dampness can soak the skin and stagnate, resulting in edema of limbs. The water is wet and stagnant, the triple energizer is out of control, and the bladder gasification is abnormal, so the urine is short, the water is wet and the skin is overflowing, so the swelling is getting worse and worse. I didn't mean to. The spleen is wet and sleepy, and Yang Can can't stretch, so it shows symptoms such as weight, mental fatigue, chest tightness, anorexia, and general nausea. White and greasy fur and slow pulse are also signs that dampness is better than spleen deficiency. Damp pathogen is sticky pathogen, which is not easy to change suddenly, so the course of disease is longer. [8]

7.6.3 Treatment: strengthen the spleen and eliminate dampness, and promote yang and diuresis [8].

7.6.4 Five-skin Drink with Yang-water Wet Immersion Syndrome [Remarks] Five-skin Drink (Chinese Tibetan Scripture), Mulberry Bark, Pericarpium Citri Tangerinae, Ginger Peel, Pericarpium Arecae, Poria, Ling Wei Decoction [Remarks] Sansheng Powder (Confucian housework): Guati, Fangfeng and Veratrum. Moisturize and diuresis with mulberry bark, aged orange peel, areca peel, poria peel and ginger peel in front; Atractylodes macrocephala and Poria cocos are used to strengthen the spleen and eliminate dampness in the rear; Rhizoma Atractylodis and Cortex Magnolia Officinalis are used for eliminating dampness and strengthening spleen; Polyporus umbellatus and Alisma orientalis induce diuresis to reduce swelling; Cinnamon warms the sun and turns gas into water. If swelling or even wheezing occurs, Herba Ephedrae, Semen Armeniacae Amarum and Semen Lepidii can be added to disperse the lung and promote horizontal wheezing. [8]

7.7 Damp-heat accumulation syndrome Yang edema [1] refers to damp-heat accumulation, watery skin, especially edema, tight skin, abdominal distension, chest tightness, irritability, shortness of breath, dry stool, short yellow urine and red tongue.

7.7. 1 The symptoms of damp-heat accumulation syndrome of yang water are edema, tense and bright skin, chest tightness and fullness, vexation and thirst, short and red urine, or constipation, yellow and greasy fur, and deep and thin pulse [8].

7.7.2 Syndrome analysis: The pathogen of water-dampness turns into heat, or the pathogen of damp-heat is trapped between tunnels in the skin, so the body is swollen and the skin is tight and bright. Due to damp-heat blocking triple energizer, qi movement is abnormal, so chest tightness and flatulence are seen. If the pathogenic heat is excessive and the body fluid is consumed, you will see polydipsia, short and red urine and dry stool. Yellow and greasy fur, heavy pulse or excessive pulse are all manifestations of damp-heat. [8]

7.7.3 Treatment of damp heat [8].

7.7.4 Prescription for Treating Syndrome of Damp-heat Accumulation of Yang Water [8]

Shuchi Shaoyin [Remarks] Shuchi Shaoyin (an effective treatment prescription for world famous doctors): Phytolacca acinosa, Alisma orientalis, kidney bean, pepper, Akebia Akebia, Poria, areca peel, ginger, notopterygium root and Gentiana macrophylla. Notopterygium root and Gentiana macrophylla in the prescription disperse wind and penetrate the exterior, so that the exterior moisture can relieve exterior sweat. Use areca nut skin, Poria cocos skin and ginger skin with Notopterygium root and Gentiana macrophylla to remove skin moisture. Alisma orientalis, Akebia Akebia, Zanthoxylum bungeanum and Phaseolus vulgaris are used together with Phytolacca acinosa and Areca catechu to facilitate defecation and take away the water evil in the house from the bottom. Dredging exterior is beneficial to dredging interior, which helps to dredge exterior, separate exterior from interior, relieve diarrhea, clear the evil of damp-heat, and relieve swelling by itself.

If the abdomen is not full and the stool is unclear, it can be combined with Ji Jiao Li Huang Pill [Remarks]: Stephania Tetrandra, Zanthoxylum bungeanum, Lepidium and rhubarb, which can help diarrhea and drain the water of stool.

If the swelling is serious, and you can't lie flat at rest, and the pulse is strong, the water is in the chest, and the lung qi is forced by the lung, it is appropriate to clear the lung and move the water. Wuling powder [Remarks] Wuling powder (Treatise on Febrile Diseases) can be used: Guizhi, Atractylodes macrocephala, Poria, Polyporus, Alisma orientalis and Wuling powder [Remarks] Wuling drink (Remarks)

If damp-heat persists for a long time, it can also turn dryness into yin injury, so it has the image of edema and yin injury. One is water retention and edema, the other is loss of body fluid, dry mouth and throat, and constipation. At this time, nourishing yin can help the evil of water, and benefiting water can also hurt yin, so it is quite difficult to treat, and it can be treated and considered. Zhu Ling Decoction in Treatise on Febrile Diseases (Remarks) can be used: Polyporus, Poria, Alisma orientalis, Ejiao and Talc, Polyporus, Poria, Alisma orientalis and Talc can clear away evil water, and Ejiao can nourish yin and blood.

If the pathogen of damp-heat pours into the bladder, injuring blood collaterals, and symptoms such as dysuria and hematuria can be seen, blood-cooling hemostatic drugs such as Cirsium japonicum and Imperata rhizome should be added as appropriate.

8 Edema Edema (edema [1][2]) is the name of the disease [5][4]. Published Su Wen's Hydrothermal Theory in Huangdi Neijing. Also known as water vapor [5]. In ancient times, it was called water, water vapor, water disease and water swelling [4]. It is a general term for diseases characterized by skin edema [1]. The patient's body fluid stays and overflows the skin, causing edema of eyelids, head, face, limbs, abdomen, back and even the whole body. Severe cases may be accompanied by pleural effusion and ascites [8]. An edema syndrome is a manifestation of systemic gasification dysfunction. The main pathological changes are lung, spleen and kidney dysfunction, unfavorable bladder gasification and edema. According to clinical manifestations, it can be divided into two types: yang water and yin water. Exogenous pathogens, the onset is more urgent, and most of them start from the head and face, and the swelling is above the waist. The location of the disease is mostly in the lung and spleen, mostly due to excess and yang water; The onset of internal deficiency is slow, generally starting from tarsal bones, and the swelling trend is below the waist. Spleen and kidney, mostly deficiency syndrome, are yin water [8][9][4].

The treatment methods of edema include sweating, diuresis, expelling, strengthening spleen, warming kidney, reducing turbidity and removing blood stasis. If the common methods are ineffective, or there are signs of blood stasis, the method of promoting blood circulation and removing blood stasis can be used [8]. The above methods can be used alone or in combination, and can be selected according to the needs of the disease [8]. Acupuncture has a certain curative effect on the syndrome, and has a good effect on improving symptoms, strengthening physique and reducing recurrent attacks.

Edema in the internal medicine of traditional Chinese medicine can be seen in modern medical diseases such as cardiogenic (such as congestive heart failure) edema, nephrotic (such as acute and chronic glomerulonephritis) edema, hepatopathic (such as cirrhosis) edema, endocrine disorders, malnutrition edema [8][9].
