The disease location belongs to the Shaoyang meridian, so the internal organs are the liver and gallbladder.
The liver is located under the hypochondriac, and its meridians are distributed on both sides of the body. The gallbladder is attached to the liver, and its pulses also follow the hypochondrium. Therefore, hypochondriac pain is mostly related to liver and gallbladder diseases. Depressed emotions, stagnation of liver qi, overeating, excessive drinking, chronic illness, fatigue, worry, or trauma from a fall can all lead to hypochondral pain. When distinguishing syndromes, the deficiency and excess of qi and blood should be distinguished first. Generally, those with qi stagnation usually have swelling and pain, and the pain area wanders indefinitely. People with blood stasis usually experience stinging pain, and the pain is localized. The hypochondriac pain of deficiency syndrome is often dull and dull, while the hypochondriac pain of syndrome syndrome is often sudden and severe.
The following syndrome types are common clinically.
① Hypochondriac pain due to liver qi stagnation. Symptoms include hypochondriac pain, erratic walking, chest tightness, nausea, thin coating and stringy pulse. Treatment should be to soothe the liver and regulate qi, and use Bupleurum Soothing Gan Powder.
②Chock pain due to qi stagnation and blood stasis. Symptoms include stabbing pain in the flanks, which may not move, lumps may be palpable under the flanks, a dark purple tongue, and a heavy and astringent pulse. Treatment should be to remove blood stasis and unblock meridians, and use Xuanfuhua Decoction with added flavor.
③ Hypochondriac pain of liver and gallbladder damp-heat type. Symptoms include hypochondriac pain, chest tightness, bitter mouth and anorexia, or yellow urine, body heat, yellow and greasy coating, and stringy and rapid pulse. Treatment should be to clear away dampness and heat, and use Longdan Xiegan Decoction.
④ Hypochondriac pain due to deficiency of liver yin. Symptoms include dull hypochondriac pain, dry mouth, upset, dizziness, red tongue with little coating, and stringy, thin or rapid pulse. The treatment is suitable for nourishing yin and softening the liver.
